1. 自身抗體
自身抗體(Autoantibody,AAB)是指人體免疫系統(tǒng)產(chǎn)生的針對自身組織的抗體,通常包括天然自身抗體和病理性自身抗體。天然自身抗體主要為 IgM型,由未突變的 V(D)J 基因編碼,對自身抗原親和力較低。天然自身抗體提供了機(jī)體抵御感染的第一道防線,具有一定的生理功能,比如維持細(xì)胞功能和調(diào)節(jié)免疫系統(tǒng)穩(wěn)態(tài)。相比之下,由體細(xì)胞突變的、和自身抗原有著高親和力的 IgG 型自身抗體則反映了一種病理過程,可能導(dǎo)致正常細(xì)胞被清除、抗原-受體的信號傳導(dǎo)或細(xì)胞效應(yīng)功能的紊亂,從而引起自身組織的病理性損傷。
2. 自身免疫性疾病及其類型
自身免疫性疾病(Autoimmune Disease, AID)是指免疫系統(tǒng)對自身細(xì)胞或成分產(chǎn)生免疫應(yīng)答而引起的疾病狀態(tài)。自身免疫性疾病的具體原因尚不完全清楚,可能與遺傳因素、環(huán)境因素以及免疫系統(tǒng)的異常反應(yīng)等多種因素有關(guān)。目前已知的自身免疫性疾病超過100種,主要分為器官特異性AD和系統(tǒng)性AD兩種類型:
(1)器官特異性自身免疫疾?。∣rgan-Specific Autoimmune Diseases):這類疾病的免疫反應(yīng)主要針對特定的器官或組織。常見的特異性自身免疫疾病包括:自身免疫性甲狀腺疾病、銀屑病、多發(fā)性硬化癥、自身免疫性肝炎、重癥肌無力、尋常天皰瘡、I型糖尿病等。
(2)系統(tǒng)性自身免疫疾?。⊿ystemic Autoimmune Diseases):相比之下,系統(tǒng)性自身免疫疾病涉及多個器官和組織,其免疫反應(yīng)波及全身多個系統(tǒng)。一些常見的系統(tǒng)性自身免疫疾病包括:系統(tǒng)性紅斑狼瘡、類白塞病、類風(fēng)濕性關(guān)節(jié)炎、炎癥性腸病、硬皮病、系統(tǒng)性血管炎等。

3. 自身抗體與自身免疫性疾病
在某些自身免疫性疾病中,自身抗體可能在疾病發(fā)作前就存在,且表現(xiàn)出顯著的特異性,因此可作為關(guān)鍵生物標(biāo)志物助力相關(guān)疾病的治療研究。在重癥肌無力或天皰瘡等器官特異性自身免疫性疾病中,自身抗體直接結(jié)合并損傷靶器官[2]。在血管炎、腎小球腎炎以及系統(tǒng)性紅斑狼瘡等系統(tǒng)性自身免疫性疾病中,自身抗體與游離分子(例如磷脂)以及細(xì)胞表面和核蛋白抗原發(fā)生反應(yīng),形成致病性抗原-抗體(免疫)復(fù)合物。這些自身抗體通過參與補(bǔ)體的 FcγR 激活以及 Toll 樣受體的內(nèi)化和激活來損傷組織和器官[3]。
4. 自身抗體與腫瘤
在腫瘤發(fā)展的早期階段,盡管腫瘤抗原在血清中很難檢測到,但人體免疫系統(tǒng)會產(chǎn)生與腫瘤相關(guān)的自身抗體,這些自身抗體有望成為癌癥早期診斷的有希望的生物標(biāo)志物。P. Budde等人[4]對接受了prostvac 和 ipilimumab治療的前列腺癌患者進(jìn)行了自身抗體分析,研究發(fā)現(xiàn),一些AAB與治療后出現(xiàn)的免疫相關(guān)不良事件(irAEs)有關(guān),而另一些AAB則可能有助于抵消炎癥分子的作用,從而減少irAEs的發(fā)生。此外,許多抗TAA自身抗體被驗證能夠作為肺癌、肝細(xì)胞癌等癌癥早期檢測的潛在生物標(biāo)志物[5,6]。
